New Patient Intake

At Momentum Health, we've streamlined the process for new patients. We now send you a link to your intake forms via email through our patient portal on Jane App. Please check your email or log into your patient portal after scheduling to ensure you have access to your intake form prior to your appointment. We recommend new patients arrive 15 minutes early to allow time for parking and to settle in comfortably at our office.
